Run DVD2AVI and load your VOBs. Tell me if on the preview window, your source displays as 4:3, even though the movie is 16:9 wide screen
That's what I've found on my movie that fails. On the ones that display 16:9 and are actually wide screen 16:9, the final size is always on target.

I've tried Kwags suggestion with Clip frame/Crop settings in TMPGEnc and processed the .d2v directly.
I used a resolution of 720x576 instead of 528x576:
