Restoring Video with Tsunami (TMPGEnc) Plus Encoder
Guide for understanding and using the various video filters found in TMPGEnc. Easily the best home software solution for repairing video. 

Restoring Video with Canopus Procoder
Guide for understanding and using the various filters found in Procoder. However, it should be noted that TMPGEnc offers more/better filters, and Procoder tends to encode better with clean sources. 

Restoring Video with AviSynth

GoldWave is very powerful audio restoration tool. In fact, too powerful. Because it has a tendency to leave digital artifacts, this method is ONLY suggest for severe audio problems, ones where noise was more dominant than the good audio.

Restoring Audio with DartPro
Another very powerful restoration tool, best used for files that have strong hiss. This method is ONLY suggested for hiss problems that cannot be corrected in SoundForge.
