To wrap this up it turned out to be a fault on...
To wrap this up it turned out to be a fault on the PCB with the delay line on it - most likely the brown potted hybrid sub boards (which are not available). I have seen this fault a couple of times...

It could be the tape itself. Certain brands of VHS tapes had problems with leaders snapping, so I wouldn't be surprised if it happened on 8mm tapes. Keep in mind that plastic gets brittle over time...
