Skew Error/Flagging with DPS 475 (1/2" open reel)

Hi all - long time lurker, first time poster.

My set up is this:

SONY AV3650 -> Leitch DPS 475 -> Blackmagic UltraStudio HD Mini -> vrecord (Mac)

I have noticed on some 1/2" open reel tapes...
