MKV DVD rip has missing audio?

Hi All.
Just trying to back up a DVD.
MKV works to Rip it to MKV files but 1 file out of 4 has the audio missing. All files rip the same no changes.
Have you any ideas why?

Tried to test burn a few DVD's but am getting Blocked images....(MKV File)
Juddering image (mp4 File) Good sharpness.

Using Mac 10:14:2 Mojave Toast Titanium 15.

Is there an idiots guide on here to show step by step how to duplicate/back up using HandBrake/MKV and Toast? or am I going about it totally the wrong way. The information on MKV web site not too helpful and not a lot on HandBreak either.
I am not a tech person and the more I read the more I get confused.
If you can take the time to walk me through the procedure or point me in the right direction I would be grateful.

Are you actually ripping?
Or also reencoding to new format? Because it seems like the latter.

Handbrake is terrible software, makes lots of bad assumptions, often butchers DVDs, when converted to H264/etc.
