How to connect video source (RCA) to video card

Hi! I am a bit of a beginner and so I apologize if this post seems trivial in any way. I am currently trying to send video from some old VHS tapes I have from an S-VHS machine by way of RCA cables into an ATI All-In-Wonder X800 video card I bought. I will be using VirtualDub to capture the video on my PC. I have installed the ATI video card but am not sure what's the correct way to connect my RCA cable (Red, White, Yellow) into the video card, which is installed into an expansion slot on my PC. The back of the video card looks exactly like the following picture:

I recognize the two ports to the right as being a VIVO and VGA output. It is obvious I need a splitter cable, but I am not really sure how to proceed at this point. I am not sure what the far left output is additionally. Could anyone lend a hand to me? Thank you!

Unfortunately, the cable you need is rare/expensive and proprietary to the PCIe All-in-Wonder series.

From left to right in your photo: the proprietary jack, DVI out, and VGA out.
