Start by reading the guides on the front page at www.kvcd.net (right panel)

It's basically the same process as if you were doing it from a DVD. Open your MPEG with DVD2AVI, create a .d2v project file, and proceed from that point just like if you had created a .d2v from a DVD.
So any guide that explains the DVD->KVCD process will work, but basically skipping the ripping process.
If you have problems opening the MPEG files on your CD, use VCDGear to extract them.

Keep in mind that the result will not be very good
You're recompressing an already compressed material. So don't expect very good results
If I were you, I would keep the VCDs/SVCDs as they are.

As it is not recommanded at all to make SVCD->KVCD conversion, it is not very usefull to spend time making such tool.
But you can use any tools that can do KVCD automatically from a d2v project (Tok, ACP). You just have the first step to do manually (build the d2v project qwith DVD2AVI).

Well, you can compress the 2 CDs down to 1, but as I said, quality will suffer.

NO
You encode the MPEG to MPEG.
If you convert MPEG->AVI->MPEG, that's the worse thing you can do

Okay, let's tell you taht in words you can understand : each time you encode a video, you LOOSE a lot of things.
The more step you have, the more are the LOSS.
When your movie was turned into SVCD, there where a lot of LOSSES.
If you convert them into KVCD, there will be others LOSSES.
That's why it is NOT recommanded to convert your SVCD into KVCD.
That's why you MUST use an intermediate avi file !
Capisce ?

IF and only IF your source is avi.
If your source is a DVD, then you convert straight to MPEG, and forget an intermediate avi conversion.
