We just have to find out why.

I have never experienced loss of frames with mencoder.
I've done it with makeAVIS and with direct VOB feed and everything I did is in full sync.
I didn't do many full encodes but...

Phil, AFAIK the only way to encode D2V file with MEncoder is using makeAVIS. Is that what you are asking EM?
BTW I have never seen frame skipping with MEncoder.
