It's possible to buy 90 min CDs here, they're a bit more expensive, but it doesn't increase the quality of a standard VCD, because standard VCD has a fixed BitRate (1150 CBR). But it can increase the storage time. A 80 min disc can keep up to 80 min of Video in a Standard VCD. I found out that there's no way to keep up with the standard and get high quality, specially if your source is very noisy (VHS) and has lots of action (home movies). You can increase the quality a bit using the notch matrix and the GOP of 18, but you still get lots of artifacts and the only way to get high quality is go KVCD and VBR.
Even SVCD is not a good choice since many player don't support (Sony and Toshiba, for instance) and it can't keep 1 hour of good quality video (I think the limit is between 45 and 50 minutes for an 80 min CD). In fact I am starting to offer KVCD as option to my clients and already created a test disc.

So change your GOP to the above, and make sure you use the notch matrix. Then you'll be making quality VCDs second to none

So, do I set to 1-5-2-1-18(NTSC) or 1-24-2-1-18(NTSC)?

They are both the same, because the last number (MAX frames per GOP) is what determines the GOP size. So 18 will be the MAX with the settings above.
