I've already done three movies with Mediator, using it's internal IVTC, and even though the begining of the movie has a short NTSC Interlaced piece, everything works perfectly! I'm not sure what would happen if the movie was a mixed FILM/NTSC, but I suppose that the result would be the same if processed with DVD2AVI.
If the movie was mixed, I would use a AviSynth script tailored for mixed NTSC/FILM, and disable "Inverse Telecine" in Mediator. There's a sample script on that over at doom9.
